Sponsor Certificate Request For Current Saint Leo the Great Parishioners

- Congratulations on being chosen to be a Sponsor for Confirmation or a Godparent for Baptism. It is such an honor!
  The following are the Guidelines for a Sponsor Certificate/Letter of Eligibility:
  You MUST be a CURRENT registered Parishioner of Saint Leo’s to request this certificate. If not, please request it from the Parish you currently attend.
  
  Please Note: If you were previously registered as a child under your parents and have reached the age of 21+,
  you are considered an adult and should be registered independantly from your parents.
  
  You must be a registered practicing Parishioner of Saint Leo the Great for a minimum of SIX (6) MONTHS in order to be eligible to receive the form from us and meet the following criteria:
  You may have one or two Godparents (baptized Catholics) for Baptism.
  One Sponsor is required for Confirmation
  If there are two Godparents, one must be male and the other female. (Canon Law)
  Godparents/Sponsors must be practicing Catholics who have received the 3 Sacraments of Initiation – Baptism, Holy Eucharist, and Confirmation and if married, be in a valid Catholic Marriage. (Canon Law)
  Godparents/Sponsors must be 16 years of age or older. (Canon Law)
  Parents cannot be Godparents/Sponsors to their own children.
  A Christian non-Catholic, who has been baptized, may be designated a Christian Witness for Baptism together with a Catholic. (A Catholic cannot be a Christian Witness)
  A non-baptized person cannot be a Godparent, Sponsor, or a Christian Witness.
