Sponsor Certificate Request For Current Saint Leo the Great Parishioners

- Congratulations on being chosen to be a Sponsor for Confirmation or a Godparent for Baptism. It is such an honor!
The following are the Guidelines for a Sponsor Certificate/Letter of Eligibility:
Being a Godparent or Confirmation sponsor is a privilege reserved for practicing Catholics, that is, those who attend Mass weekly. Everyone is asked to track their weekly attendance by using their envelopes. If you are an electronic giver, it is as easy as putting the packet in your glove compartment and using your empty envelope to track attendance. If you are asked to be a sponsor, Father Dan will be required to put in writing that you are a weekly communicant. Since he is on the wide learning curve to learn names, thanks in advance for your cooperation.

You MUST be a CURRENT registered Parishioner of Saint Leo’s to request this certificate. If not, please request it from the Parish you currently attend.

Please Note: If you were previously registered as a child under your parents and have reached the age of 21+,
you are considered an adult and should be registered independently from your parents.

You must be a registered practicing Parishioner of Saint Leo the Great for a minimum of SIX (6) MONTHS in order to be eligible to receive the form from us and meet the following criteria:
You may have one or two Godparents (baptized Catholics) for Baptism.
One Sponsor is required for Confirmation
If there are two Godparents, one must be male and the other female. (Canon Law)
Godparents/Sponsors must be practicing Catholics who have received the 3 Sacraments of Initiation – Baptism, Holy Eucharist, and Confirmation and if married, be in a valid Catholic Marriage. (Canon Law)
Godparents/Sponsors must be 16 years of age or older. (Canon Law)
Parents cannot be Godparents/Sponsors to their own children.
A Christian non-Catholic, who has been baptized, may be designated a Christian Witness for Baptism together with a Catholic. (A Catholic cannot be a Christian Witness)
A non-baptized person cannot be a Godparent, Sponsor, or a Christian Witness.

If you feel that you qualify under the above guidelines, please complete the following. We will contact you after verifying the information you have provided regarding approval and receiving the Sponsor Certificate/Letter of Eligibility
